Failures rarely begin with a catastrophic decision. More often, they begin quietly. Warning signs become familiar; small deviations get explained away, and experienced teams slowly accept increasing risk. Mike Ciannilli spent 30 years inside the U.S. space program, supporting 58 Space Shuttle missions and serving as a NASA Test Director for 21 launch countdowns. After helping recover Columbia and her crew following the 2003 disaster, Mike created and led NASA’s Apollo, Challenger, Columbia Lessons Learned Program.

Today, Mike takes audiences inside launch countdowns, mission operations, post-incident investigations, and the Test Director discipline developed in environments where failure carried irreversible consequences. His presentations combine operational storytelling, humor, and the realities of decision-making under pressure to show audiences how capable organizations drift toward preventable mistakes and what disciplined leaders do differently to interrupt that process early. Mike is the recipient of NASA’s Silver Snoopy Award, presented by astronauts for outstanding contributions to flight safety and mission success. He speaks from operational responsibility, not hindsight.

Preventing Failure: From Lessons Learned to Lessons Applied

Why do smart, experienced organizations keep repeating the same mistakes?

Drawing on three decades inside NASA and the hard-earned lessons of Apollo, Challenger, and Columbia, Michael Ciannilli takes audiences inside mission operations, launch countdowns, and post-incident investigations to reveal how small warning signs are ignored and why lessons learned often fail to become lessons applied. Combining compelling storytelling, humor, and practical principles, he shows leaders how to create a culture where critical insights drive better decisions and prevent repeat failures.

Audience Takeaways include:

Understand why intelligent, data-driven organizations still repeat the same mistakes.

Recognize the warning signs that teams often normalize under pressure.

Learn how to turn hard-earned lessons into disciplined operational change.

Strengthen communication and decision-making before small problems become major failures.

Build a culture that values learning, accountability, and continuous improvement.

Apply practical tools to ensure important lessons are remembered—and acted upon.

This keynote can be customized for leadership teams, safety and risk professionals, operations organizations, and any audience responsible for making critical decisions in complex environments.
